Registration Information

Welcome to T’siya Day School! We are so excited to have you join our amazing community. Our school is dedicated to providing a safe, nurturing, and academically challenging environment for our students and their families. We strive to create a learning environment where children can thrive academically, socially, and culturally. A place where students will have high expectations of themselves, be responsible and respect traditional values. We invite you to learn more about our school, our staff, and our programs before enrolling your child in our school.

Registering Your Child

Parents/guardians must provide ALL necessary administrative records before their child can be registered at T’siya Day School. Please thoroughly review the application for a complete list of necessary registration forms. Any students desiring to register at T’siya Day School will need to have a parent/guardian accompanying them at the time of registration.

New Students

Please complete the following forms below. All forms must be filled in full.

Please attach a copy of the following documents:

  • Current Immunization Records
  • Student’s Birth Certificate
  • Certificate of Degree of Indian Blood (BIA Form 4432). Students applying for admission must demonstrate membership in a federally recognized tribe, or at least one-fourth total degree Indian blood direct descendant of an enrolled member of a federally recognized tribe and provide a Certificate of Indian Blood (CIB).

Returning Students

Please complete the following forms below. All forms must be filled in full.

Please attach a copy of the following documents:

  • Immunization Records. Each student must provide an up-to-date immunization record.

Is any other information helpful to the process?

Upon admission, any student with a known medical issue, including a food allergy, must disclose the information to the Principal and registrar. School officials will ensure that this information is shared with appropriate staff within the school in accordance with the school’s policy and make the necessary referrals to the school’s Section 504 Coordinator.

What documents do I need bring to register my child?

In addition to the items listed above, the following documents may need to be furnished:

  • Students that reside outside of the attendance boundaries must have an out-of-boundary waiver that has been approved by the school board.
  • If you are enrolling a child and you are not the biological parent you will need to provide proof of guardianship by presenting one of the following:
    • Legal court documents
    • Care Givers Affidavit
    • Power of Attorney
  • If transferring from another school, we require the official transfer records from the previous school.

What is the birthday cutoff for Kindergarten?

Students registering in Kindergarten must be five (5) years old by September 1st of the current year.
